New Patient Registration

Any person living within the Practice area may register as a patient by presenting their Medical card to reception , or by completing a Patient Registration Form. You may choose to see any Doctor or nurse in the practice but it is better to try and see the same doctor consistently. No guarantee of availability of any particular member of the practice team can be given.

Patients have a responsibility to keep appointments or to inform the practice in advance if they are unable to attend. Children should generally be accompanied by a responsible adult.

Patients will only be removed from the Practice List in exceptional circumstances; however violence or aggression towards any member of the Practice Team will not be tolerated.
